Please answer all questions below. This form must be filled out in its entirely to be considered for an adoption of a Silky Silhouette Shih Tzu puppy/adult.

I understand that owning a Shih Tzu is a major commitment, Shih Tzu are NOT outdoor dogs. They are social dogs and require attention from people and can NOT tolerate temperature extremes. They require grooming that can cost $30-$40 a month and need to be combed everyday or two to prevent matting. Shih Tzu will need vaccinations,worming,teeth cleaning, ear care, anal gland attention, and continued vet care. I, the buyer, am willing and able to take on this responsibility. The seller requires $250.00 deposit to be applied to: reserving a puppy from Silky Silhouette Shih Tzu.
If you are mailing a payment, please send a USPS Postal Money Order by Priority Mail with Delivery Confirmation. No personal checks can be accepted. Unless other arrangements are made, the deposit must be made within 5 business days, or the puppy will go back up for adoption. Please be sure you can accept the responsibility of a puppy as deposits are non-refundable under ANY circumstances!
I understand the deposit will be applied towards the purchase price. I, as the buyer, shall incur all expenses in relation to shipping. Generally shipping costs run between $275.00-$300.00, dependent upon buyer location and airline used. Buyer assumes all costs associated with shipping. Additional costs
when shipping include health certificate and crate and airline ticket. Unless other arrangements are made, shipping costs or mileage must be added into the balance due.
Puppies are generally ready to be shipped between the ages 8 to 10 weeks. I understand, that the seller reserves the right to determine when the puppy is ready to be shipped. I need to have my puppy shipped/ I will be picking up my puppy (circle one).
All puppies are sold with a spay/neuter agreement and NOT to be used for breeding purposes, unless other arrangements have been made. The puppy will come limited registration/The puppy will come with Full AKC breeding and Show rights. I as the buyer understand that if I want Full AKC breeding rights, that I am to call the breeder, before I send this form, and be able to provide references in order to be granted Full registration. I as the Purchaser agree to keep the said puppy at my home and away from public places(vet visits allowed) until after the third and final puppy vaccination. Applications will be considered in the order they are received.
The balance is due by the time your puppy is 7 weeks old and must be paid by cash or USPS Postal Money Order. deposit is forfeited, contract is void, and the puppy will be placed back up for adoption.
This form must be filled out, signed and mailed to me,WITH the deposit.

Any disputes shall be litigated in Athens, AL. County of Limestone.
